It is never too late to get the smile you want. Age is not a factor.
Dean’s case is one I think about when patients tell me they have left it too long. That it is too late. That they are at a point in their life where doing something about their teeth feels like it no longer makes sense.
I disagree with that view completely. And Dean is a good reason why.
He came to see me about the missing teeth on his upper arch. He was not a young man making a cosmetic decision. He was someone who wanted his smile back and had decided that wanting it was reason enough to do something about it. That attitude made him one of the easiest patients I have worked with. He knew what he wanted and he was ready to do what was needed to get there.

Multiple missing upper teeth and the daily awareness that comes with them.
Missing teeth on the upper arch are among the most visible and most impactful presentations I see. Every conversation, every meal, every moment where a smile would be natural, there is an awareness that something is not right. Patients develop habits around it. The careful way they hold their mouth. The smile that does not quite open. The photographs they step out of.
Dean had been living with multiple missing upper teeth for some time before he came to see me. He had reached the point where the situation was no longer something he was willing to accept as permanent. He wanted his full smile back. He wanted to stop thinking about his teeth in every situation where his confidence should have been completely free.
He had done his research before coming in. He knew about dental implants, he understood broadly what the process involved, and he arrived at the consultation ready to have an honest conversation about what was possible for his specific case.
- Planning the treatment
Going through the options and building a plan that was right for Dean's case.
At the consultation we reviewed Dean’s upper arch in detail. The number of missing teeth, the condition of those remaining, and the bone available for implant placement were all assessed. The treatment plan that emerged was for multiple implants on the upper arch using the Osstem dental implant system, followed by a full set of permanent crowns on the anterior teeth once healing was complete.
I use Osstem implants across both my Northwich and Wilmslow practices. I have been a Key Opinion Leader for Osstem UK for over four years and have presented long-term clinical evidence on their success rates. They are the system I trust with my own patients because the evidence behind their long-term performance is exactly what I need to stand behind the results I promise.
Dean’s treatment was planned to replace each missing tooth in the upper arch individually and restore the natural appearance of the full smile, including the front teeth, with permanent porcelain crowns designed to match his existing teeth in colour and shape.

Implants placed. Twelve weeks of healing. Permanent crowns designed and fitted.
The implants were placed in a single surgical appointment under local anaesthetic. Following placement, we waited 12 weeks for the implants to fully integrate with the jawbone through the process of osseointegration. During this healing period Dean had temporary restorations in place so there was never a visible gap.
After the 12-week healing period was confirmed, we moved into the design and fabrication stage for the permanent restorations. The crowns on the anterior teeth, the ones most visible when Dean smiled, were given particular attention. Getting the shape, shade, and proportion right on the front teeth is the stage that most determines whether a patient can smile fully and freely, and it is the stage I spend the most time on.
We went through the final try-in and review stages together before the permanent crowns were fitted. The finished result was exactly what we had planned.

- The result
Cannot stop smiling. Receiving compliments every single day.
Dean came back to see us for his final review a few weeks after the permanent crowns were fitted. What he told me is something I hear in different forms from patients whose smile has been properly restored, but it never gets less satisfying.
He said he could not stop smiling. That he had been receiving compliments on a daily basis. That people who knew him had noticed the change immediately and that he had found himself explaining, more than once, what the treatment involved.
That is the outcome that matters. Not the clinical documentation of what was placed and when. The patient who cannot stop smiling and wants everyone to know why.
There is something particularly meaningful about a full smile restoration in a patient who, like Dean, had perhaps wondered whether the right time had already passed. The right time is when you decide you want it. That is what Dean’s journey demonstrates.
- Dr Nikhil reflects
Age has never been a reason to say no
One of the things I say to patients who tell me they feel they have left it too long is this: the implants do not know how old you are. The bone, the gum, the clinical conditions for successful treatment, these are what I assess. Age alone has never been a reason to say no.
Dean’s case is a reminder that the decision to do something about your smile is valid at any stage of life. The confidence that comes from a complete, natural-looking smile is not a young person’s privilege. It belongs to anyone who wants it and is willing to take the first step.
Doing a full smile restoration is always life-changing treatment. It increases confidence and brings something lighter into the lives of everyone around the patient too. Dean is a perfect example of that.
“For every problem, there is a solution. It might be challenging, but it is almost always possible.”
