All-on-4 Dental Implants by Dr Nikhil Oberai

If you are missing most or all of your teeth, or living with dentures that have never really worked for you, there is a permanent fixed solution. For many patients it can be completed in a single day.

Living without teeth, or with dentures, takes more from you than you realise

There is a version of this that builds slowly. A tooth here. A tooth there. A denture that was supposed to be the answer but never quite felt like one. And eventually you find yourself at a point where eating, speaking, and smiling in public all carry a weight they did not used to.

The patients I see for All-on-4 have often been through a lot before they get to me. Failed treatments. Dentures they hate. Years of managing rather than living. And almost without exception they tell me the same thing after treatment. They wish they had come sooner.

Does any of this sound familiar?

Eating has become genuinely difficult

Properly chewing food, not just soft foods but a proper meal, has become something you either struggle through or avoid entirely. You have adjusted what you order, what you cook, and what you accept as normal. None of it should be normal.

Your dentures give you constant low-level anxiety

Will they move? Will they slip in conversation? Will they shift when you laugh or bite into something? That anxiety becomes background noise, always there, never quite gone. It changes how present you can be in social situations.

You feel older than you are

When teeth are missing and the jawbone is receding, the face changes shape. The structure that teeth and roots provide is gone, and the result is visible over time. It is not vanity to want that addressed. It is a genuine health consideration.

You have been told you do not have enough bone

This is the most common reason patients with extensive tooth loss are turned away. They arrive at my practice having already heard that implants are not possible for them. In the majority of cases I see, that is not the whole story.

The good news is, All-on-4 can give you a completely fixed smile, often in a single day.

All-on-4 is a full arch reconstruction technique where a complete set of fixed teeth is supported by just four strategically placed implants. The implants are angled to maximise the use of available bone, which means many patients who have been told they lack the bone for conventional implants are in fact suitable for All-on-4.

Where it is clinically possible, I carry out the entire procedure in a single appointment. Any remaining teeth are extracted, the four implants are placed, and a temporary fixed bridge is fitted the same day. You arrive without teeth that work. You leave with a fixed, functional smile.

I have performed this procedure many times at both my Northwich and Wilmslow practices. The transformation it makes to people’s lives is unlike anything else I do. Patients who had stopped eating what they wanted, stopped smiling in photos, stopped feeling like themselves, they leave a completely different person.

What All-on-4 actually means, in plain English

The name refers to the number of implants used: four per arch. Four titanium posts are placed into the jawbone, two at the front and two angled towards the back where bone density is typically higher. That angled placement is what makes All-on-4 so effective, particularly for patients with some degree of bone loss.

Once the implants are in, a bridge of ten to twelve teeth is attached on top. In the Smile-in-a-Day protocol, a temporary version of this bridge is fitted on the day of surgery, meaning you leave with fixed teeth from the very first appointment. Several months later, once the implants have fully fused with the bone, the permanent bridge is fitted.

The result is a full set of teeth that are permanently in your mouth. You brush them. You eat with them. You forget they are implant-supported rather than natural, because for all practical purposes, they function identically.

Every stage is designed to be clear and pressure-free. Nothing moves forward until you are completely comfortable and ready to proceed.

1
Starting point

Free 3D Smile Assessment

Your consultation is completely free and includes a full cone beam CT scan of your jaw. That scan tells me precisely how much bone you have, where it is most dense, and exactly how the implants should be positioned for the best long-term result. You leave with a written treatment plan and a complete cost breakdown. No obligation at all.

Full Case Assessment and Planning

Before surgery is booked, I review your CT scan in detail and plan the precise placement of all four implants. I walk you through every stage of the day itself, the temporary bridge you will leave with, and the timeline through to your permanent teeth. Sedation is available throughout the procedure if that would help.

The procedure

Extractions and Same-Day Implant Placement

On the day of treatment, any remaining teeth that cannot be retained are removed and all four implants are placed in the same appointment. The procedure is carried out under local anaesthetic, sedation is available, and typically takes three to four hours. This is the Smile-in-a-Day protocol: one long appointment that changes everything.

Temporary Fixed Bridge Fitted the Same Day

Before you leave, a temporary fixed bridge of ten to twelve teeth is attached to the implants. It is not the permanent version, that comes later, but it is fixed, it is functional, and it looks like a complete smile. You walk out with teeth. That is not an exaggeration.

Healing and Integration

Over the following three to four months, the implants fuse fully with the jawbone. You carry on with your normal life throughout. I see you at regular intervals to monitor the healing, and my team is available at any point if you have questions.

Your Permanent Bridge is Fitted

Once the implants have fully integrated, your permanent fixed bridge is designed and fitted. It is crafted to match the exact shade, shape, and proportion that suits your face, and it is built to last. This is the final appointment. The one where the work is done.

Here is what my patients tell me after treatment

These are not features from a brochure. These are the things patients describe when they come back for their first check-up after the permanent bridge is fitted.

They eat a proper meal for the first time in years

Steak. Crusty bread. Apples. Anything on the menu. The restrictions they had built their life around, the soft foods, the careful chewing, the avoided dishes, simply stop applying. Patients frequently tell me this single change is worth everything.

They stop thinking about their teeth

With dentures, the teeth are always somewhere in the background of your mind. Will they move? Are they visible? Am I chewing on the right side? With All-on-4, that constant low-level monitoring stops. The teeth are just teeth. They do what teeth do.

Their face starts to look like their face again

When the jawbone is stimulated by implant roots rather than left to recede beneath a denture, the structural support that teeth provide is restored. Patients notice the difference in how they look, and so does everyone around them.

They have their confidence back

This is the one that comes up in almost every post-treatment conversation I have. Not just the smile, the confidence to be fully present in social situations. To eat with other people. To laugh without thinking. To feel like themselves again.

Nothing to remove, clean separately, or worry about

No soaking overnight. No adhesive. No separate cleaning routine. The bridge is brushed exactly like natural teeth with a standard toothbrush and dental floss. That is the entire maintenance requirement.

How much does a single dental implant cost in the UK?

It is almost always the first question, and it deserves a direct answer.

All-on-4 dental implants at my practices in Northwich and Wilmslow start from £11,500 per arch. That means one jaw, upper or lower. If both arches are being treated, I offer the Platinum Package from £19,500 for both.

The honest caveat is this: the exact cost depends on your specific case. How many teeth need removing. The density and volume of available bone. Whether any additional preparatory work is needed. A price given without examining your CT scan is not a real quote. Your written, personalised cost plan is confirmed at your free 3D Smile Assessment before you commit to anything.

I'm Dr Nikhil Oberai MSC & Dental Implants are all I do

All-on-4 is not a procedure that should be placed with someone who does it occasionally. It requires precise surgical planning, a thorough understanding of bone anatomy, and the experience to handle complex cases when they arise. I hold a Master’s Degree in Dental Implantology from the University of Manchester and have been performing full arch reconstructions for over a decade. I teach the same MSc programme as an Honorary Clinical Teaching Fellow, which means the techniques I use are the same ones being taught at postgraduate level.

I also regularly take on All-on-4 cases that other practices have turned away. Patients with more significant bone loss, patients whose treatment abroad has failed, and patients who have been told that full fixed implants simply are not possible for them. In most cases I find there is a route forward. I will always tell you honestly what that is before you make any decision.

“For every problem, there is a solution. It might be challenging but it is almost always possible.”

Common questions my patients ask me about All-on-4

Here are the questions our All-on-4 patients ask most often — answered honestly and in plain language.

All-on-4 at my practices in Northwich and Wilmslow starts from £11,500 per arch. Both arches are available from £19,500 as a Platinum Package. Your exact written cost is confirmed at your free 3D Smile Assessment before any commitment is made. 0% finance through Tabeo is available.

Quite possibly yes. All-on-4 was specifically designed to work with reduced bone volume by angling the rear implants to use the denser bone that is typically available further back in the jaw. Many patients who have been turned away for conventional implants are suitable for All-on-4. The free 3D CT scan gives a definitive answer for your specific anatomy.

A full denture is removable, requires adhesive, allows the jawbone to continue shrinking beneath it, and restricts what you can eat. All-on-4 is fixed permanently in your mouth, stimulates the jawbone to prevent bone loss, and allows you to eat anything. The maintenance requirement for All-on-4 is identical to natural teeth, standard brushing and flossing.

Smile-in-a-Day is the protocol where extractions, implant placement, and the fitting of a temporary fixed bridge all happen in a single appointment. Where it is clinically appropriate, I perform All-on-4 this way at both my Northwich and Wilmslow practices. You arrive without teeth that work. You leave with a fixed, functional smile the same day.

The procedure is carried out under local anaesthetic and sedation is available if that would help you feel more comfortable. Most patients report that the day itself was more manageable than they had anticipated. There is typically some swelling and soreness for the first few days following surgery, which is managed with standard pain relief and aftercare guidance from my team.

From your free assessment to the fitting of your permanent bridge, total treatment time is typically four to six months. The surgical day itself takes three to four hours. The healing period before the permanent bridge is fitted takes approximately three to four months, during which you will have your temporary fixed bridge in place.

Yes, this is a case type I treat regularly. Patients who have had implant work carried out overseas that has subsequently failed or caused problems. I carry out a full assessment, develop a corrective plan, and in most cases am able to provide a long-term stable solution. Come in for an assessment before drawing any conclusions about what is or is not possible.
