Smile in a Day Dental Implants by Dr Nikhil Oberai
One appointment. A complete, fixed set of teeth. For patients who are ready to stop managing and start living, Smile-in-a-Day changes everything, and it can happen sooner than you think.
You have been living around this problem for long enough
There is a particular kind of exhaustion that comes with living without a functional smile for a long time. It is not dramatic, it is quiet and cumulative. The foods you stopped ordering. The photos you stepped out of. The conversations you managed rather than enjoyed. The sense that this is just how things are now.
Most patients I see for Smile-in-a-Day have been in that place for longer than they should have been. Some were told implants were not possible. Some assumed the cost was out of reach. Some simply did not know that a single-day solution existed. All of them, without exception, tell me afterwards that they wish they had come sooner.
- Why patients come to see me
Does any of the points below sound familiar?
You have no functional teeth, or very few left
The remaining teeth are failing, or already gone. What you have is not a smile, it is a situation you manage each day. Eating, speaking, and appearing in public all carry a weight they should not.
You are wearing dentures and they have never felt like a real answer
They move. They restrict what you eat. They require adhesive. They come out at night. You have accepted all of this as the reality of your situation, but it does not have to be.
The idea of a long, multi-stage treatment feels overwhelming
You know something needs to be done but the thought of months of appointments, of being in between stages, of having a visible gap during treatment, it has stopped you from starting. I understand that. Smile-in-a-Day exists precisely because of it.
You have been told you do not have enough bone for implants
This is the single most common reason patients with extensive tooth loss arrive at my practice having already heard no from somewhere else. It is not always the whole picture. A precise 3D scan of your jaw tells us what is actually possible, not what has been assumed.
The good news is, you can leave my practice the same day with a fixed, complete smile.
Smile-in-a-Day is not a marketing phrase. It is a clinical protocol in which all remaining teeth are removed, implants are placed, and a fixed temporary bridge is fitted, all in a single appointment. You arrive at the practice without teeth that work. You leave the same day with a full set of fixed teeth.
That temporary bridge is not a denture. It is not removable. It is attached to the implants and it functions like real teeth from the moment it is fitted. Several months later, once the implants have fully fused with the bone, the permanent fixed bridge is placed, and the treatment is complete.
I have performed this procedure many times at both my Northwich and Wilmslow practices. The moment a patient sees their temporary smile for the first time, on the day of surgery, is something I never get tired of witnessing. It is why I do this work.

What Smile-in-a-Day actually means
Smile-in-a-Day is built on the All-on-4 protocol. Four titanium implants are placed per arch, two at the front and two angled towards the back where bone is typically denser. That angled placement is what makes the technique so effective even for patients with some degree of bone loss, because it uses the bone that is available rather than requiring the bone that is not.
The procedure is carried out in a single long appointment under local anaesthetic, sedation is available throughout. Any remaining teeth are carefully extracted. The four implants are placed. And before you leave, a fixed temporary bridge of ten to twelve teeth is attached on top.
Over the following three to four months, the implants fuse with the surrounding bone through a natural process called osseointegration. You carry on with your life as normal throughout, with fixed teeth. Once healing is complete, the permanent bridge is designed, crafted, and fitted. That is the end of the treatment journey.

On the day of your procedure, everything happens in a single appointment. Any remaining teeth that cannot be retained are carefully removed. Four titanium implants are placed into the jawbone using the All-on-4 protocol. Before you leave, your temporary fixed bridge, a complete set of ten to twelve teeth, is attached to the implants. The procedure takes three to four hours. You came in without a functional smile. You leave with one.

Over the next three to four months, the implants fuse with the surrounding bone. This process, osseointegration, happens naturally while you carry on with your normal life. Your temporary fixed bridge is in place throughout. I monitor your healing at regular check-ins and my team is available at any point.

Once healing is confirmed, we move to the design stage for your permanent bridge. Shade, shape, and proportion are all chosen to suit your face and your smile. This is a collaborative process. I want the finished result to look and feel exactly right for you.

Here is what my patients tell me after treatment
These are not clinical outcomes from a data sheet. These are the things patients actually say at their review appointments, the changes that matter to real people.
They leave the surgery with teeth on the day
This sounds obvious but the reality of it lands differently. Patients who came in that morning without a functional smile leave the same afternoon with a fixed set of teeth. The emotional weight of that moment is not something I can fully describe. You have to experience it.
The anxiety about their teeth simply stops
With dentures or extensive missing teeth, the background noise of anxiety about your mouth is constant. With fixed implants, it disappears. Patients describe this as one of the most unexpected and significant changes, more significant, even, than the physical result.
They eat anything they want, immediately
The temporary bridge placed on the day of surgery is functional from the moment it is fitted. Within the appropriate healing guidelines, patients can eat normally. The permanent bridge, once fitted, has no food restrictions whatsoever.
Their face looks like their face again
Implants stimulate the jawbone the way natural roots do. The facial structure that teeth provide is restored. Patients and the people around them notice the difference, not just in the smile, but in the overall appearance of the face.
There is nothing to remove, manage, or worry about
No overnight soaking. No adhesive. No separate cleaning routine. The bridge is brushed exactly like natural teeth. That is the sum total of the daily maintenance requirement.

I'm Dr Nikhil Oberai MSC & Dental Implants are all I do
Smile-in-a-Day requires a very specific combination of surgical experience, advanced planning, and the clinical confidence to manage complexity in real time. It is not a procedure for someone who places implants occasionally. I hold a Master’s Degree in Dental Implantology from the University of Manchester, have been performing full arch reconstructions for over a decade, and teach advanced implant techniques at the same institution as an Honorary Clinical Teaching Fellow.
The cases I take particular satisfaction in are the ones that arrived with the most doubt. Patients who were told the procedure was not possible for them, patients whose bone loss was described as too severe, patients whose previous treatment elsewhere had failed. In the vast majority of cases I find a viable path forward. I will always tell you honestly what I find before you make any decision.
“For every problem, there is a solution. It might be challenging but it is almost always possible.”

Common questions my patients ask me about Smile-in-a-Day
Here are the questions our smile-in-a-day patients ask most often — answered honestly and in plain language.
Smile-in-a-Day at my practices in Northwich and Wilmslow starts from £11,500 per arch on the Gold Package. Both arches start from £19,500 on the Platinum Package. Your exact written cost is confirmed at the free 3D Smile Assessment before any commitment. 0% finance through Tabeo is available.
Yes. The Smile-in-a-Day protocol means that extractions, implant placement, and the fitting of a fixed temporary bridge all happen in a single appointment. The temporary bridge is attached to the implants before you leave. It is not a denture, it is fixed. You cannot remove it. You leave with a complete, functional smile on the day of surgery.
Quite possibly. The All-on-4 protocol uses angled implants to maximise the bone that is available, which means many patients who have been turned away elsewhere due to bone loss are in fact suitable for Smile-in-a-Day. The free 3D CT scan gives a precise answer for your specific anatomy. I would rather you came in and found out for certain than assumed the answer was no.
It is carried out under local anaesthetic and sedation is available throughout. The procedure itself typically takes three to four hours. In the days following surgery there is some swelling and tenderness, which is managed with pain relief and detailed aftercare guidance from my team. Most patients tell me the experience was more manageable than they had anticipated.
From your free assessment to your permanent bridge, treatment takes approximately four to six months. The surgical day itself takes three to four hours. The healing period before the permanent bridge is fitted takes three to four months, during which your fixed temporary bridge is in place throughout.
Conventional implant treatment for a full arch is staged over several months with a gap between the implant placement and the final restoration. Smile-in-a-Day compresses this into a single day for the surgical stage, you leave with fixed temporary teeth on the day of the procedure. The permanent bridge follows after healing, as it would with any full arch reconstruction.
Yes, I treat this regularly. Patients who have had full arch work placed in the UK or abroad that has subsequently failed or caused problems. I carry out a full assessment, develop a corrective plan, and in most cases am able to provide a long-term stable solution. Come in for a proper assessment before drawing conclusions about what is or is not possible.
