A denture was supposed to solve the problem. For most patients, it has not.

There is a conversation that happens regularly in dental practices across the UK. A patient has lost teeth. The options are discussed. And somewhere in that conversation, a denture is presented as the straightforward, affordable, sensible solution. What is not always fully explained is what life with a denture actually involves. Or what else is available.

Patients who come to see me about denture alternatives have usually been wearing one for months or years. They have adapted to the limitations because they were told there was nothing better available to them. In the majority of cases, there is.

It moves when you eat, speak, or laugh

The shifting, the slipping, the moment of self-consciousness when you realise the denture has moved. The adhesive that was supposed to prevent this but does not quite manage it. Patients describe this as the single most exhausting aspect of wearing a conventional denture. The constant low-level monitoring of something that should simply work.

You have accepted a significantly restricted diet

Hard foods, chewy foods, anything that tests the stability of the denture. You have quietly stopped ordering or cooking certain things so long ago that it barely registers any more as a restriction. It has simply become normal. It does not have to be.

Many patients accept a conventional denture not because it is genuinely the best option for their situation, but because no one presented the alternatives clearly, or because they assumed the alternatives were out of their reach financially. The good news is, a conventional denture is rarely your only option.

For most patients who are currently wearing a conventional denture, or who have been told that a denture is what they need, at least one implant-based alternative exists. In many cases several do. The right one depends on the clinical picture and on what matters most to the individual patient in terms of stability, permanence, and cost.

The alternatives range from implant-retained dentures, which use two to four implants to anchor a removable denture securely in place, through to fully fixed implant teeth that are permanently in the mouth and never need removing. The difference between a conventional denture and any of these alternatives is significant. Not every alternative is right for every patient. Here is an honest overview of the main options, what each one involves, and who it is best suited to.

Implant-Retained Dentures

Two to four titanium implants are placed in the jaw. The denture has corresponding attachments on its underside that clip firmly onto these implants when inserted. The result is a denture that is mechanically secured rather than relying on suction and adhesive. It is still removable for cleaning, but it does not move when you eat, speak, or laugh. No adhesive required. This is the most accessible implant-based alternative in terms of cost and clinical requirements. It is a significant improvement on a conventional denture in every practical respect.

Fixed Implant Bridge (Multiple Missing Teeth)

Where several adjacent teeth are missing, a fixed implant-supported bridge replaces them permanently. Two or three implants support a fixed bridge of natural-looking teeth. Nothing is removable. Nothing moves. The adjacent healthy teeth are left completely untouched. This is the appropriate option where gaps are localised rather than covering a full arch.

Four implants support a complete arch of fixed teeth. The teeth are permanent. They never leave their mouths. They are cleaned like natural teeth. They handle the same biting forces as natural teeth. And in many cases the entire procedure, including a temporary fixed bridge, is completed in a single day. For patients who are currently wearing a full upper or lower denture and want a permanent fixed solution, All-on-4 is the most significant quality of life improvement available.

Zygomatic Implants

An implant-retained denture requires less bone than a full arch reconstruction. Zygomatic implants bypass the upper jaw entirely for patients with severe upper jaw bone loss.
